To ensure your visit will be as pleasant as possible, please keep the following in mind:
Before your appointment date:
- Please complete the registration form and health history, and email them to us or bring them with you to your appointment. Please list all medications and health conditions.
- Watch the specified video for your surgery all the way to the end so the system confirms it was completed (if applicable).
- Confirm your appointment. You will receive a call from our office to review instructions and confirm your appointment prior to your appointment day. If we leave a message for you to call our office, please make sure you return our call. Due to a high rate of unconfirmed appointments failing to appear, we do not guarantee unconfirmed appointment times, and they may be given away to other patients with urgent or emergent needs. 48 hours notice is required for appointment cancellations. Feel free to initiate the call to confirm your appointment 2-3 days prior.
On your appointment date:
- Wear a short-sleeved shirt, as we routinely monitor your blood pressure and heart rate during all surgical procedures. We would like you to feel as comfortable as possible.
- Remove all jewelry and piercings prior to arrival and have one fingernail on each hand without nail polish or acrylic fingernails for monitoring your oxygen levels.
- Please arrive on time for your scheduled appointment, or we may not have the time needed to complete your appointment.
- If the patient is under the age of 18, a parent or legal guardian must be present.
- If the patient has a legal guardian or POA, they must be present at the appointment with a copy of the guardianship/POA paperwork.
- Bring photo identification (e.g., driver’s license).
- Bring medical and dental insurance cards.
- Prepare for payment as discussed when scheduling the appointment. Payment will be collected prior to surgery.
If you have a WageWorks credit or debit card (for your HSA or FSA account), they do not allow our office to charge these directly. You will need to pay with cash, bank debit, or credit card the day of service and then submit the receipt to them for reimbursement.
Thank you for choosing North Dakota Oral Surgery & Dental Implant Center! We look forward to making your visit as pleasant as possible.
